This procedure – essentially a medical tattoo for the areola – uses specialized pigments to recreate a natural-looking nipple and areola. Understanding Areola Tattooing
Areola tattooing (also known as areola micropigmentation) is typically the final step in breast reconstruction. It’s a form of areola pigmentation after mastectomy. Some people get surgical nipple reconstruction first, then add tattooing; others use tattooing alone as a less invasive alternative.

The Areola Tattooing Process: What to Expect
A typical areola tattoo appointment involves three stages:
1. Consultation & Planning: You’ll first have a consultation to make sure you’ve healed enough (typically a few months post-surgery) and to discuss your goals. You’ll decide on the desired color, size, and placement of the tattoo.
2. Tattoo Procedure: The tattoo session usually takes about 1–2 hours for one or both nipples. A numbing cream is often applied for comfort. Using a sterile needle device, the artist implants pigment in a circular design and adds shading to create a 3D effect. Even though the tattoo is flat, careful shading makes it look like a realistic, raised nipple.
3. Aftercare: Once the tattooing is done, the area will be cleaned and covered with a bandage. The new tattoo will look darker at first – this is normal. You’ll get instructions on caring for the area over the next couple of weeks.
Healing and Aftercare
Like any tattoo, an areola tattoo needs proper care to heal well. For a smooth areola tattoo healing process, it’s important to follow recommended aftercare. You can expect some redness or mild swelling for a few days. Most of the surface healing is done in about two weeks, and the tattoo settles into its final color over the next month.
Key aftercare tips include keeping the area clean, moisturized, and protected:
- Clean Gently: Leave the initial bandage on as instructed (about 24 hours). After removing it, wash your hands before touching the tattoo. Gently clean the area with mild soap and water, then pat dry.
- Moisturize: Apply the recommended ointment or moisturizer a couple of times a day to keep the skin from drying out. This helps protect the area and prevent infection. Avoid touching or picking at any scabs as it heals.
- Avoid Water & Sun: Until healed, avoid submerging the area in pools, hot tubs, or baths, and keep it out of direct sunlight. (Even after healing, use sunscreen on the area to help preserve the tattoo’s color.)
By your follow-up visit (around 6–8 weeks later), the area should be fully healed. If needed, the artist can do a touch-up then to refine the color or shape.
Areola Tattoo Cost and Insurance
In the U.S., areola tattooing typically costs a few hundred dollars for one side or around $600–$800 for both.
By law, insurance plans that cover mastectomy must also cover all stages of reconstruction, including nipple and areola restoration. This means if your areola tattoo is done as part of your reconstruction through your surgeon or hospital, your insurance should pay for it. (If you go to an independent artist outside the medical system, insurance may not cover it, so check in advance.)
If insurance won’t cover it, ask about payment plans or charities that help fund areola tattoos for survivors. Don’t let cost be a barrier to getting this empowering service.
